Post-inflammatory hyperpigmentation (PIH) is one of the most common skin concerns we treat at Mountcastle Medical Spa & Laser Center. Whether it’s triggered by acne, laser treatments, or minor injuries, PIH can leave stubborn dark spots—especially on deeper skin tones.
The good news? With a medically backed and personalized plan, PIH is highly treatable.
Here’s our step-by-step approach to achieving clearer, more even-toned skin.
Step 1: Comprehensive Skin Assessment
Every treatment begins with a one-on-one consultation at our Ashburn med spa with one of our licensed providers—such as our award-winning aesthetic injectors or expert aestheticians.
We evaluate:
- Fitzpatrick skin type
- The source and depth of pigmentation
- Recent skincare products or treatments
- Whether inflammation is still present (active acne, eczema, etc.)

Step 2: Prep the Skin with Medical-Grade Skincare
Before in-office treatments, we prime your skin using a customized skincare regimen. This allows the skin barrier to heal and prepares it to respond better to pigment correction.
Products we may recommend include:
- Brightening agents: hydroquinone, kojic acid, arbutin, and azelaic acid
- Retinoids: to promote cell turnover
- Antioxidants: like iS Clinical Pro-Heal Serum
- Broad-spectrum sunscreen: like Colorescience Face Shield SPF 50

Step 3: In-Office Treatments to Fade PIH
We tailor your treatment plan based on your skin’s tone, sensitivity, and lifestyle. Some of the best options for PIH include:
Chemical Peels
We offer several options, including:
- The Perfect Derma™ Peel: A medium-depth peel safe for all skin types
- Mandelic or lactic acid peels for sensitive or darker skin tones
These peels work by gently exfoliating the top layers of skin and targeting melanocytes (the pigment-producing cells).
Microneedling with PRF or Brightening Serums
Microneedling triggers collagen production and allows for deeper penetration of targeted serums. At Mountcastle, we often enhance this with:
- PRF (Platelet-Rich Fibrin) to naturally speed up healing
- Brightening serums rich in tranexamic acid or vitamin C
Laser Treatments (For Select Skin Types)
- Clear + Brilliant®: A gentle resurfacing laser for mild hyperpigmentation
- BroadBand Light (BBL) (external link): Targets pigment safely and effectively
Laser treatments are selected with extreme care, especially for skin of color. Safety always comes first.
Step 4: Post-Treatment Aftercare & Sun Protection
After any in-office treatment, your skin will be more sensitive to UV exposure. Proper aftercare is essential to prevent rebound pigmentation.
We recommend:
- Daily use of mineral-based SPF 50, like Colorescience Total Protection
- No exfoliating or retinoids for at least 5–7 days
- Avoiding direct sun, including through car windows or overcast days
Pro tip: Wearing a wide-brimmed hat and using a tinted mineral sunscreen (external link) offers added protection for melanated skin.
Step 5: Maintenance & Results
Results begin to appear after 4–6 weeks of treatment and continue to improve with consistent skincare and maintenance. For long-term results, we may recommend:
- Quarterly chemical peels
- Monthly facials or gentle microneedling sessions
- Prescription pigment inhibitors when needed
Most patients see significant improvement in tone, clarity, and confidence.
